Medical Assistance. Application for entitlement recognition and other procedures (REM) < Go back

This service allows workers in the Special Scheme for Seafarers to request in their own name, as a representative or proxy, via Electronic Register, entitlement to medical assistance, both for the holder and their beneficiaries. It also allows you to claim reimbursement for health treatment in a European country or free pharmacy for minors with a degree of disability of more than 33%.

Who it is for

For workers in the Special Scheme for Seafarers and their beneficiaries.

What you can do

Submit an Electronic Register application.
Download and attach the completed application form.
Attach documentation.

What you need to do it

Fill in and attach the application form.
Attach copies of all documents detailed in the instructions.
Have software that enables you to fill in and save the application form to be attached.

Please note

If you are logging in with a permanent Cl@ve Password, in order to guarantee a higher level of security for this service, the login validation will include sending an SMS to the mobile number that you provided at the time of registration. You will need to enter the code sent in the SMS in the login section.

If you are acting as a representative, you must attach the standardised representation form, which can be found in the accompanying documents section, or any legally valid document that is proof of the representation.

In the case of acting as power of attorney, you must be registered in the Electronic Registry of Powers of Attorney.

The processing province is the one to which you wish to submit your application.

Attached documents must not exceed 10 Mb.

Next steps

Once the process has been completed, you will receive a “proof of receipt” in PDF format.

If you do not receive the confirmation message, or if you receive an error or failed sending message, you should resubmit the application at another time or use other channels.

The Social Marine Institute will send the decision regarding the application to the address of the interested party.
