Request for issue of specific initial and advanced health training certificates (REM) < Go back

This service allows you to apply, via Electronic Register, on your own behalf or on behalf of another person, for the issue of specific health training certificates regulated under Order PRE/646/2004, of 5 March, which establishes the minimum content of specific health training programmes and the conditions for issuing and standardising health training certificates for sea workers.

About this procedure

Who it is for

People who have completed the courses regulated by Order PRE/646/2004 of 5 March and in the Resolution of 24 April 2013, of the Social Marine Institute, on the minimum contents of the refresher programmes in specific health training and on specific health training online, passing the theoretical and practical tests established in accordance with their objectives.

Holders of "Advanced diploma in supervision and control of machinery and ship installations" or "Advanced diploma in navigation, fishing and maritime transport" or equivalent, issued by the competent Education Administrations, may apply for the initial Specific Health Training Certificate through this route.

Holders of diplomas awarded by the Higher Educational Centres of the Merchant Navy or Faculties of the Merchant Navy or equivalent, issued by the competent Administrations, may apply for the Advanced Specific Health Training Certificate in this way.

To people or entities applying for the issue of these certificates on behalf of the above.

  • Download and attach the completed application form.
  • Attach documentation.

What you need to do it

  • Fill in and attach the application form.
  • Attach copies of all documents detailed in the instructions.
  • Have software that enables you to fill in and save the application form to be attached.

Please note

If the application relates to the issuing of a health training certificate in accordance with the provisions of the second and third additional provisions of Order PRE/646/2004 of 5 March, a copy of the qualification you wish to validate must be attached to the application. The corresponding provincial directorate of the Social Marine Institute will require originals of the necessary documents

If you are logging in with a Username + Password (permanent Cl@ve), in order to guarantee a higher level of security, you will be sent an SMS code to the mobile number that you provided at the time of registration. You must enter this code in order to sign the communication in the Electronic Register.

To act on behalf of a third party, you must have a power of attorney registered in the Electronic Power of Attorney Registry that allows you to carry out this procedure. If you do not have a valid power of attorney in the aforementioned register, you must provide proof of representation by attaching the standard form of representation that can be downloaded for completion in the "attached documentation" section, or by attaching any other legally valid document that provides a reliable record of this.

The total size of the documents to be attached must not exceed 10 Mb.

Next steps

Once the process has been completed, you will receive a "proof of receipt" in PDF format.

If you do not receive the confirmation message, or if you receive an error or failed sending message, you should resubmit the application at another time or use other channels.

Issuance by the provincial directorate of the Social Marine Institute of the relevant specific health training certificate or issue of the negative decision on it.
