Through this service you can apply via the electronic register, and on behalf of another person, for entitlement to medical assistance, both for the holder and for their beneficiaries.

What you can do

  • Submit an application via the Electronic Register for entitlement to medical assistance on behalf of another person.
  • Download and attach an application form.
  • Attach documentation.

What you need to do it

  • Fill in and attach the application form and the standardised representation form.
  • Attach copies of all documents detailed in the application form instructions.
  • Have software that allows you to fill in and save the application form to then be attached.

Please note

If you are logging in with a Username + Password (permanent Cl@ve), in order to guarantee a higher level of security, you will be sent an SMS to the mobile number that you provided at the time of registration. You will need to enter the code sent in the SMS in the login section.

The application form was initially designed to be submitted in paper at the Regional INSS Office, so there are references that do not correspond to the online service.

The processing province is the one to which you wish to submit your application. The remarks section should only be completed when an application form is not attached.

Your connection has a time limit. Therefore, if your session times out while filling in the application form you will have to restart the session and complete the items pending.

The total size of the documents to be attached must not exceed 10 Mb.

Next steps

Once the procedure is processed, you will receive "proof of receipt" (zip) in the form of an application receipt in PDF format and the signature of the server, guaranteeing that the transaction has been correctly processed.

If you do not receive the confirmation message, or if you receive an error or failed sending message, you should resubmit the application at another time or use other channels.

The INSS will send the resolution of the application to the interested party's address.
