Submission of medical reports < Go back
This service allows you to submit by electronic registration, in your own name or on behalf of another person, updated medical reports, addressed to the Medical Units of the National Social Security Institute, in relation to the benefits of the Social Security system, except for the confirmation and medical discharge reports for Temporary Disability, which must be submitted through the service "Management of the temporary disability benefit". In the case of Temporary Disability initiated abroad, it can also be used to submit medical reports/parts.
Who it is aimed at
To those who, on their own behalf or on behalf of another person, wish to submit by electronic registration updated medical reports addressed to the Medical Units of the National Social Security Institute for the assessment of the benefit requested, as well as to submit by electronic registration updated medical reports/parts for the recognition of a temporary disability started abroad.
In Catalonia, the documentation must be sent to the Catalan Health Service (Catalan Institute of Medical Evaluations), which is the competent body in the Autonomous Community.
What does it allow you to do
- Attach updated medical reports addressed to the National Social Security Institute.
What you need to do it
- Attach any medical reports you wish to send.
- Please indicate in the "REMARKS" section the benefit for which you are providing the medical reports. If the temporary disability began abroad, you must indicate this in this section.
- Have an active file for the benefit indicated in "OBSERVATIONS" in the province of the National Social Security Institute to which the reports are addressed, except in the case of a request for recognition of temporary disability abroad.
- If you are acting as a representative, you must attach the standard form of representation, which appears in the attached documentation section, or any other legally valid document that provides reliable proof of representation.
To be taken into account
If you access with User + Password (permanent Cl@ve), in order to guarantee a higher level of security, a code will be sent via SMS to the mobile phone number you declared during the registration process, which you must enter in order to be able to sign the submission in the electronic register.
The province of processing indicates the Provincial Directorate of the National Social Security Institute to which you wish to send your documentation. In the case of Temporary Disability started abroad, the province of processing will be that of the contribution account code of the company in which you are registered as a worker.
In Catalonia, the documentation must be sent to the Catalan Health Service (Catalan Institute of Medical Evaluations), which is the competent body in the Autonomous Community.
The connection session is temporarily limited, so if it expires while you are filling in the application you will have to start it again and finish the pending actions.
The total size of the documentation to be attached must not exceed 10 Mb.

You will receive an "acknowledgement of receipt" in PDF format once you have completed the procedure.
If you do not receive the confirmation message or, as the case may be, you receive an error or transmission failure message, the submission must be made at another time or by other means.
